My brother is autistic and we have been getting him the Guest Assistance Pass for years now. It really helps A LOT. Prior to getting it, he would be in tears, wanting to leave at noon because he just couldn't handle it.
Here's what you do to get one. You go to City Hall with the child and tell them you'd like an assistance pass. They'll ask if they are able to handle stairs and such. They also ask you what will happen if they have to wait in line. We always tell them he'll have meltdowns, because that's about the best way to describe it. Then they give you a card that you'll show to the cast member in front of each ride to find out where you're supposed to go. Each ride it's different. Sometimes it's through the exit, sometimes it's through the fast pass line. It really depends, so don't assume. We also have a doctor's note, just in case. We've only had to use it a couple times at City Hall, but it's good to have it because you really never know how the cast member there is going to be about it. Also, when the day's over we always keep the pass so that we can just show them exactly what we need the next time.
